ADVANCED PRIMARY CARE MODEL (APCM)

APCM is a new Medicare – supported program that helps us provide more coordinated and proactive care.  It’s part of a national effort to improve health and reduce hospital visits for Medicare patients. We have always focused on this model of care, but Medicare is now recognizing the importance of following patients closely and the benefits of having a true primary care leader on your team. Medicare is rewarding providers who demonstrate this type of care with extra compensation.

The Goal of the Program:

• Focus on Prevention and Chronic Care – helping you stay healthier longer
• Better Coordination – Communicating with all members of your care team and sharing information and data to help guide your care
• Extra Support – On call service 24/7, access to telemedicine visits, referrals to specialists when needed

Key Points:

• Your Medicare benefits stay the same
• You can still see any Medicare provider
• There are no extra costs to you outside of your routine Medicare obligations

If you agree that ATX Primary Care is your primary source of care under the APCM, please sign below.  You are acknowledging:

• I know that I still have the right to see any provider who accepts Medicare, but only 1 provider can furnish the APCM services
• Signing this form will not affect my Medicare benefits and I may stop agreeing to these services at any time
• Cost sharing may apply according to the benefits in my Medicare plan
